Homemaker & Personal Care Services (HPC)

GentleBrook’s HPC program allows individuals with developmental disabilities to stay in their homes, but still receive support with activities of daily living, as well as social and community inclusion. This program is available through the level one Medicaid waiver, SELF waiver, and Individual Options (IO) waiver.

Services included in the waiver:

  • Homemaker/Personal Care: Assistance with daily living activities such as personal grooming, incontinence care, and medication reminders.
  • Transportation: Rides to and from doctors appointments, community & leisure activities, and grocery stores, and banks.
  • Informal Respite: Care provided in a person’s home, the home of a friend, or family member, to provide relief for the primary caregiver who normally cares for the individual.

Our Providers

Our direct care providers are specifically trained in the unique needs and requirements of people with developmental disabilities. In addition to specialized training, our providers are all certified in CPR and First Aid. They also much complete a background check, drug test, and a driving record check.

Eligibility

Individuals with developmental disabilities who meet the criteria for the care given in an Intermediate Care Facility (ICF) but choose to live in their community.
